In the Season 11 finale of “Chicago Fire,” Mouch, portrayed by Christian Stolte, a fixture on the series since its debut in 2012, faced a harrowing situation. While responding to an incident involving an active shooter, Mouch was struck by shrapnel. Following a frantic trip to the hospital, the firehouse received a reassuring update: it wasn’t a bullet, and Mouch was awake and on the road to recovery.
Nevertheless, in the concluding seconds of the episode, Herrmann (played by David Eigenberg) remained at Mouch’s bedside when he noticed an alarming change. Mouch’s complexion had paled considerably. Herrmann’s gaze shifted downward, revealing that Mouch was bleeding profusely—and then, his vital signs flatlined. The camera gradually pulled away, and the significance of the episode’s title, “Red Waterfall,” became apparent as blood cascaded down the edge of Mouch’s hospital bed, while Herrmann urgently called out for a doctor.

While the season finale saw the return of Jesse Spencer, who reprised his role as Matt Casey after departing the show following ten seasons, Taylor Kinney’s Kelly Severide did not make a comeback. Kinney had taken a leave of absence earlier in the season to attend to a personal matter, with his character embarking on an arson investigation training program out of town. However, during the finale, Severide’s wife, Stella (played by Miranda Rae Mayo), became suspicious that something more was afoot. Consequently, she declared her intention to bring her husband back home.
Furthermore, Casey dropped a hint that Spencer might not just be making a brief cameo, as he proposed to his ex, Brett (portrayed by Kara Killmer).
The Season 11 finale also marked the departure of showrunner and co-creator Derek Haas, who had announced in March that he would be leaving Wolf Entertainment at the conclusion of the season.
